This Agreement establishes a legal binding relationship between you and GO MD USA, LLC when you opt-in, enroll, use the ACP service, or make any changes to your account. It is essential that you understand your legal rights under this Agreement, as certain disputes may need to be resolved through arbitration rather than a court trial. GO MD USA, LLC retains the right to amend or adjust this Agreement at any time and at its sole discretion. Any such changes or modifications will be binding upon you and will be posted on the GO MD USA, LLC website at gomdusa.net. We recommend checking the website regularly to stay up-to-date with the latest changes to this Agreement.
The Service Agreement
Kindly read this Agreement for GO MD USA, LLC Affordable Connectivity Program ("ACP") Terms and Conditions of Service carefully as it constitutes a legally binding agreement between you and GO MD USA, LLC. It takes effect when you opt-in, enroll in the GO MD USA, LLC Wireless ACP service, use it, or modify your account. The Agreement outlines essential information about your legal rights and mandates that particular disputes be resolved via arbitration instead of a court trial. GO MD USA, LLC reserves the right to modify or change this Agreement at any time and at its sole discretion. Once posted on the GO MD USA, LLC website at gomdusa.net, any modifications or changes to this Agreement become binding on you. You should visit the GO MD USA, LLC website regularly for updates to this Agreement.
The service agreement comprises this Agreement, the GO MD USA, LLC General Terms and Conditions of Service, the GO MD USA, LLC ACP Terms and Conditions of Service (if you enroll in the ACP program), and any other referenced policies or documents. If there are conflicts, the order of precedence is as follows: (1) this Agreement, (2) the ACP Terms and Conditions of Service, and (4) the General Terms and Conditions of Service.
The ACP is a benefit program of the Federal Communications Commission ("FCC"), which launched on December 31, 2021, and aims to make broadband more affordable for struggling households. Eligible households may receive a discount of up to $30 per month towards broadband service, or up to $75 per month for households on Tribal lands, as defined by FCC rules. Furthermore, eligible households can receive a one-time discount of up to $100 to purchase a laptop, desktop computer, or tablet from participating providers if they contribute more than $10 but less than $50 toward the purchase price. Each household is limited to one monthly service discount and one device discount. ACP benefits are accessible from any participating provider of your choosing, and you may transfer your monthly service benefit to another provider at any time. Consumer eligibility, provider participation, product availability, and FCC and USAC approval govern the ACP benefit's availability. The discount is available each month the customer maintains ACP services and eligibility.
By subscribing or applying to GO MD USA, LLC services supported by the ACP, you authorize GO MD USA, LLC and its contracted partners to collect, use, share and retain your personal information, including information necessary to determine eligibility for the ACP program, such as full name, full residential address, date of birth, last four digits of social security number, telephone number, eligibility criteria and status, the date on which the ACP service discount was initiated and terminated (if applicable), ACP connected device distribution date/type/make and model/status, usage status, and other compliance requirements, and the amount of support being sought.
To enroll in the ACP, an applicant must meet specific eligibility requirements and have a service address in a location where GO MD USA, LLC is authorized to offer ACP service as an approved Broadband Service Provider.
The following are the methods by which your household may be eligible for the Affordable Connectivity Program (ACP):
- Your household income meets the requirements.
- You or your child or dependent participates in specific government assistance programs.
- You or someone in your household is already receiving a Lifeline benefit.
Based on Your Income
If your household income is equal to or below 200% of the Federal Poverty Guidelines (FPG) based on your state and household size, then you qualify for the Affordable Connectivity Program (ACP). Please refer to the table below for the income limits by household size, which are set at 200% of the 2022 FPG.
Household Size
48 Contiguous States, D.C., and Territories
When you apply for the ACP, you might be required to provide evidence of your income, such as three consecutive pay stubs or a tax return.
Federal Assistance Programs
To qualify for the ACP, you (or someone in your household) must participate in one of the following programs:
- Supplemental Nutrition Assistance Program (SNAP)
- Medicaid
- Special Supplemental Nutrition Program for Women, Infants, and Children (WIC)
- Supplemental Security Income (SSI)
- Federal Public Housing Assistance (FPHA), which includes Housing Choice Voucher (HCV) Program (Section 8 Vouchers), Project-Based Rental Assistance (PBRA)/202/811, Public Housing, and Affordable Housing Programs for American Indians, Alaska Natives, or Native Hawaiians
- Veterans Pension and Survivors Benefit
- Free and Reduced-Price School Lunch Program or School Breakfast Program, including at U.S. Department of Agriculture (USDA) Community Eligibility Provision schools
- Received a Federal Pell Grant in the current award year
When applying for the ACP, you may be required to provide proof of participation in one of these programs by presenting a card, letter, or official document.
Tribal Assistance Programs
If you reside on qualifying Tribal lands, you are eligible for the ACP if either your household income is at or below 200% of the Federal Poverty Guidelines, or you (or someone in your household) participate in one of the federal assistance programs listed above, Bureau of Indian Affairs General Assistance, Head Start (only households meeting the income qualifying standard), Tribal Temporary Assistance for Needy Families (Tribal TANF), or the Food Distribution Program on Indian Reservations. When you apply for the ACP, you may be required to provide proof of participation, such as a benefit letter or official document.
If You Already Receive a Lifeline Benefit
The Affordable Connectivity Program (ACP) is designed by the Federal Communications Commission to provide assistance to low-income consumers in making communication services more affordable. If you are already receiving benefits from the ACP, you are automatically eligible for the program, and you can utilize both benefits simultaneously. You have the option to apply both benefits to the same or different services. To begin receiving discounts from the ACP, you can consult your internet service provider for further details.
Qualify Through Your Child or Dependent
Participation in one of the programs listed above by any member of your household makes your household eligible for the ACP benefit. This means that if, for instance, your child or dependent takes part in the Free and Reduced-Price School Lunch Program or attends a USDA Community Eligibility Provision school, your household is qualified for the ACP benefit.
Your eligibility to take part in the ACP will be determined by the National Verifier or a State Administrator, both administered by USAC. If you are a new applicant, you must complete an ACP application and certification form and provide supporting documents to confirm that you meet the eligibility criteria for the ACP benefit.
New ACP beneficiaries who subscribe to GO MD USA, LLC ACP services will receive a SIM card that allows them to access the GO MD USA, LLC network with a compatible GSM device. They may also be entitled to a one-time discount of up to $100 on a connected device. If the beneficiary has not yet received their one-time discount, GO MD USA, LLC will provide one broadband-enabled connected device in accordance with its Handset Policy and the ACP guidelines.
One ACP Discount Per Economic Unit Rule
The ACP provides support to one economic unit per household in the form of broadband service. An economic unit refers to a group of individuals who live at the same address, share expenses and income, and are not allowed to receive ACP benefits from multiple providers. Failure to comply with this rule is considered a violation of federal regulations, which could lead to de-enrollment from the ACP and prosecution by the government. By accepting the ACP benefit, you give consent to sharing personal identification information such as your name, telephone number, and address with USAC and/or its agents to confirm that you do not receive more than one ACP benefit. In case you or anyone in your family receives ACP assistance from another telephone or internet service provider, you give consent to transferring your benefit to GO MD USA, LLC.
ACP Benefit Availability
If the FCC declares that funding for the ACP is no longer accessible, GO MD USA, LLC will make an effort to inform you 30 days in advance that its ACP-supported service will be terminated. Once the ACP is terminated, customers have the option to subscribe to one of our service plans, which are available at the time, and pay the applicable undiscounted rate along with applicable taxes and fees.
Consent to Disclosure of Information
By submitting the GO MD USA, LLC ACP application, you give your consent for the release of your information, including financial information, to our designated agent for the administration of your service with GO MD USA, LLC. This consent remains in effect even after the termination of this Agreement. You also authorize GO MD USA, LLC to communicate with or access information from representatives of state or federal agencies concerning your eligibility and participation in the ACP. GO MD USA, LLC reserves the right to review your eligibility status at any time and may request written documentation of your household's income or participation in a qualifying federal or state program.
National Lifeline Accountability Database (NLAD)
When an applicant applies for ACP benefits, they will be automatically entered into the NLAD database upon enrollment in states that mandate its use. During the application process, the applicant's name, address, date of birth, and social security number will be provided to NLAD. The address will be verified for accuracy, and the applicant's identity will be verified. The applicant's eligibility for an ACP benefit from another provider will also be checked. If the applicant is currently receiving a benefit from another provider, GO MD USA, LLC will transfer the benefit to their services only with the subscriber's consent.
Activation of Service
When you enroll in GO MD USA, LLC's ACP services as a new applicant, the device you selected during enrollment (Tablet, Mobile Device, Hot Spot) will be delivered to the address you noted in the application. Alternatively, if you apply at a retail location, the device will be given to you after the enrollment and approval process. Your GO MD USA, LLC ACP service will be activated by making an outbound call, sending a text, using data, or confirming your desire to activate your service. You must accept the telephone number assigned to your GO MD USA, LLC account at the time of activation, and you will not acquire any proprietary interest in the assigned number. Your assigned number will not be changed unless required by a carrier or if a change is necessary to alleviate harmful or harassing situations. You cannot select a specific number for your GO MD USA, LLC service unless otherwise specified in the GO MD USA, LLC General Terms & Conditions. If applicable, you may choose to port your telephone number to GO MD USA, LLC once your ACP application has been approved. For more information, please contact customer service at (833) 706-3872
Account Activity Requirement
To continue receiving your benefit, it is necessary for you to use your device to make or answer a voice call, send a text message (if your device allows), use data, purchase minutes, or respond to direct contact from GO MD USA, LLC at least once every 30-day period.
Maintaining Eligibility and Service
You will continue to receive ACP service from GO MD USA, LLC as long as you maintain your eligibility for the program and the ACP remains active. In the event that GO MD USA, LLC determines that you are no longer eligible for the ACP service, you will be notified of the cancellation and given a 30-day notice to respond. Failure to demonstrate continued eligibility will result in the cancellation of your service and the loss of any remaining benefits. If you no longer meet the requirements for the ACP discount, you are required to notify GO MD USA, LLC within 30 days of this change to be removed from the program.
ACP is Non-Transferrable
Eligibility for the ACP benefit is personal to you. You may not transfer, to any third party, any of your rights or benefits received under the GO MD USA, LLC service, including but not limited to, any voice minutes or broadband data received under GO MD USA, LLC service funded by the ACP.

The authorization period for your GO MD USA, LLC service usage is up to thirty (30) days, starting from the following dates, whichever is conclusively determined by GO MD USA Wireless at its sole discretion: (i) the date when your GO MD USA, LLC account became active; (ii) the date of the initial service transaction on your GO MD USA, LLC account; (iii) the receipt of the allotment of GO MD USA, LLC rate plan airtime; (iv) the purchase or addition of GO MD USA, LLC airtime; or (v) the date when you were deemed eligible for the GO MD USA, LLC plan. The anniversary date of your service, also known as the Service Period, is the first day of each month, and GO MD USA, LLC airtime will be added to your account in accordance with your rate plan, provided that you maintain eligibility for ACP service.

Rates, Usage and Included Allotments
As long as you meet the ACP eligibility criteria, GO MD USA, LLC will provide you with a monthly data allowance while you are enrolled in their ACP-supported services.

*LEGAL NOTES
GO MD USA LLC's Affordable Connectivity Program (ACP) is a government benefit program that offers supported services. Enrollment in ACP is only available to eligible consumers and is limited to one per household, and it is non-transferable. Proof of income or participation in a qualified program may be required to enroll. Although the program offers unlimited service, GO MD USA LLC reserves the right to terminate or modify your service if it is used in a manner that interferes with other customers' service or disproportionally uses GO MD USA LLC's network resources. GO MD USA LLC may also limit, alter, or discontinue your service if you generate unusually high call volumes, have lengthy average call times, incur abnormally high costs, or use an abnormally high amount of data compared to other customers.
Your service plan terms and conditions may limit the amount of high-speed data included in your plan and the actual speeds you achieve while using broadband services. The actual performance of your wireless data service may vary based on several factors such as reception, technology, and network traffic, and GO MD USA LLC does not guarantee the speeds of the data services provided.
Eligible households can receive one monthly service discount under the ACP. If you are already enrolled in the Emergency Broadband Benefit Program, your benefits will automatically transfer to the ACP after the transition period. When the ACP program ends or GO MD USA LLC's promotional FREE ACP service offer ends, you will be notified, and you can keep your ACP service by paying the applicable undiscounted rate plus fees and taxes. For further information about ACP eligibility and the program, visit https://www.gomdusa.com/how-to-qualify or contact us at (833) 706-3872 . Other terms and conditions may apply.
